Please complete the indemnity form below in order that your child can take part in a visit.

Please do not ask for leave of absence for a family holiday in term time if you can avoid it. If requests for holidays are unavoidable these will be dealt with on an individual basis depending on previous holidays taken and/or absences due to illness etc.
